Allow personal smart phone access to email
In the private sector many companies allow team members at any level to correspond thru email using their own mobile phone equipment. This reduces a cost for CMS not buying the phone, increases productivity of employees who choose to do so by allowing them to communicate from anywhere and mobile platforms are usually more restricted/secure than personal computers. We currently have access to webmail so let’s take it one step further.

More Efficient CMS password Synchronization
Change the CMS login requirement for changing expiring passwords to having the user go to EUA Passport instead of having the opportunity to change passwords at the desktop screen.

Clearly define the roles for the COTR and the GTL and train them each accordingly
Clearly define the roles and responsibilities for both the COTR and the GTL so that each are clear. Train the COTR's and GTL's according to the defined division of work.

Develop the rooftop areas (2nd floor) near the hallway Link areas attached to the CMS buildings.
Recommend to develop the rooftop areas (on the second floor) between the Hallway Link areas that attaches to the CMS Central, South and North buildings. The rooftop areas can be developed into a much needed outdoor lunch environment that could also possibly double as a meeting gathering area that can be used during nice weather conditions.
Since these areas can only be accessible from the links on the second floors, I can’t foresee a great need for security. I also can’t foresee a huge expense associated with making this idea happen. Other than making sure the rooftops are stable enough…

Clean up and Standardize Medicaid and CHIP data. Create CM Master Beneficiary and Provider Profiles
Create CMS Enterprise Beneficiary and Provider Profiles to include CHIP, Medicaid, and Medicare data
9 votesOIS responded that your idea is currently in the planning and early development phases. The vision of the Master Data Management (MDM) system is to use modern data engineering methods and tools to provide complete profiles of Medicare, Medicaid and Children’s Health Insurance Program data. The MDM system will offer a suite of services that will allow CMS to link and synchronize Beneficiary, Provider, and Organization data from multiple disparate sources.
Provider and Beneficiary profiles, or Books of Records, will be obtainable through a single call to a trusted, authoritative data service that is part of the MDM system. Additional cost estimates to include Medicaid/CHIP data into the MDM has been estimated to be $60Million dollars over several years.

Invest in the gym
The equipment could be upgraded and there could be sound boxes at each cardio machine to plug into to hear the televisions instead of tuning to a radio station.

Implement incentive programs where employees receive a percentage of actual cost savings realized
Create an incentive program that allows employees to receive a percentage of any actual cost savings resultant from ideas submitted/implemented. Front line employees have an incredible advantage for recognizing opportunities for process improvements and opportunities for cost reductions. Allowing them to reap part of the rewards associated with the implementation of their ideas would spur creativity and esprit de corp. We would truly all “be in it together”.

Hiring practices; External employee selection
CMS has a very long history of selecting employees based on familial relationships. It is not uncommon to have many family members working in and around CMS, however one has to wonder how all of these family members meet the KSAs or all have the experience to work here? I think that the entire Agency suffers when employees are merely selected based on nepotism. Selections made to just "get the person" in the door are not fair to the beneficiaries, providers, and other entities that we serve. Compress work schedule
Don't charge employees an hour or 2 for Holidays when on a compress work schedule. CMS is the only agency that does this.

Pursue doctors who get incentives from drug companies for prescribing their drugs for antikickback
It’s widely known that pharmaceutical companies recruit the loyalty of doctors by offering them incentives, such as well paid faculty positions for seminars held at resort locations and other valuable benefits, with the understanding that the doctors will prescribe and promote the company’s drugs. There is little difference between those practices and compensation paid to a doctor for making referrals to a radiology clinic, a clear violation of anti-kickback laws. The result of the kickbacks in both situations is to siphon money from the healthcare system for the purpose of mutual enrichment of the participants in the arrangement without contributing…
5 votesThe Anti-Kickback Statute covers payments made to physicians by pharmaceutical companies to induce the prescriptions of their drugs. OIG investigates violations of the anti-kickback statute, regardless if the payment is made by a pharmaceutical company or a radiology clinic. OIG issued guidance in May 2003 titled “OIG Compliance program guidance for pharmaceutical manufacturers” that addresses these concerns.
CMS is implementing section 6002 of the ACA, often referred to as the “sunshine” provision that will require drug companies report a variety of payments they make to physicians. The increased transparency on payments is intended to reinforce the importance of maintaining proper and legal physician relationships with industry.

Cover nontraditional services that are demonstrated to reduce treatment cost and improve outcomes.
Many studies have shown that a simple course of hypnosis before surgery allows the patient to receive a lower dose of anesthesia and can greatly reduce recovery time. The same may be true of acupuncture treatments yet there is no method of Medicare or other third party payment for them, so they have not been widely used. There is clear evidence that these treatments work although the scientific mechanism through which they work isn’t well understood. I suggest that Medicare pay for hypnosis and acupuncture as incident to physician services under approved circumstances, such as before and after surgery.

Install phone booths so staff without offices can talk without disturbing their cubicle neighbors
Some Federal agencies have small phone "booths" where staff can sit and use the phone for up to 30 minutes. This allows them to make needed personal calls or to have a quiet environment for presentations on conference calls. This both supports privacy and better cubicle etiquette.

WiFi access throughout the campus would allow staff to make better use of their laptops and take them to meetings.

Plan for overhauling the Selection, Development & Oversight of CMS Leadership
My suggestion really incorporates many previous suggestions into a larger plan for redesigning the selection, training and oversight of management within CMS. Year after year, we get our survey results which show that we are one of the worst federal agencies to work for, despite having one of the highest grade structures in the federal workforce. Management gathers and tries to determine what employees want or what perks can be provided to obtain better survey results—making CMS one of the “best places to work.” There isn’t anything wrong with this, except that the focus of the conversation is always what…
